Payment and Insurance
Payment
- I accept payment by credit card or HSA card only (no cash or checks)
- You must provide me with the complete credit card information PRIOR to your first session
- I will keep your card information on file
- I will charge your card after each session
- It might be several days after your session that your card is charged since producing an invoice can take time
- Adjustments to your charge may be necessary after receiving information from your insurance company about how much you owe (this might take up to a few weeks, as the claim is filed with Anthem and I receive a response showing your responsibility)
- You will get a receipt for each payment by email
- I am not able to offer reduced rates or payment plans
Insurance
- I take Anthem insurance or you can self-pay
- Please note that I take all Anthem commercial insurance plans (including HealthKeepers) but NOT Medicaid (HealthKeepers Plus) and NOT Medicare plans
- You must provide me with complete insurance information and a copy of your insurance card PRIOR to your first session
- I can not file out-of-network insurance under any circumstances
- You are responsible for being aware of any deductible that you owe and how much your copay or coinsurance will be (you can call the number on your insurance card to find out if you are not certain)
Statements and billing history
- I can provide you with any invoices, statements, or payment history that you might need for your records or tax purposes
